THIS IS A FIXED TERM CONTRACT COVERING MATERNITY LEAVE.
This is an exciting opportunity to be part of the Early Supported Discharge Service (ESD) for Stroke in the East Cornwall team based at Bodmin Hospital.
We are looking for a Physiotherapist/ Occupational Therapist to join the ESD team to provide a five day rehabilitation service within a Stroke survivor’s home environment, facilitating early discharge from Acute and rehabilitation Stroke Units.
This is a key post in the ESD team. Good communication, stroke rehabilitation skills, and ability to work autonomously are essential. You will also need to have good time management prioritisation skills, and flexibility to work across the East or West of Cornwall with use of own vehicle.
Candidates applying for this post must have a good grounding in post graduate rotations and have experience in stroke assessment and rehabilitation. You will need to be confident in your assessment skills, able to formulate rehabilitation programmes, evaluate goals with appropriate outcome measures, and facilitate appropriate discharge plans and onward referrals from ESD. This will include cross agency working e.g. Department of Health and Social Care, District Therapy Teams, Stroke Specialist Therapists, Stroke Co-ordinators and voluntary agencies.
You will be required to provide team training and offer supervision to more junior staff and students, assisting audit of service and data gathering as delegated by Clinical Leads.
You will be integral to the team and have a key role in its delivery and assisting the Clinical Lead in service delivery and evaluation. You will have support from the Clinical Lead in ESD and the Clinical Leads based on the Acute Stroke Unit with access to supervision and in-service training opportunities.
